Frailty is defined as ‘a long-term condition in which multiple body systems gradually lose their in-built reserves, resulting in an increased risk of unpredictable deterioration from minor events. The consequences of escalating Frailty are adverse outcomes such as disability and its consequences, frequent hospital admissions and increasing demand for long-term social care support‘- Joining the Dots: A blueprint for preventing and managing Frailty in older people (British Geriatrics Society, 2024).

This programme was developed and tested by the Frailty Network and Clinical Education Centre in collaboration with representatives from various HSC disciplines from across all Trusts, independent sector, CEC and primary care.
Frailty Learning Pathway offers a structured approach for all roles and experience levels. The programme builds awareness, skills and confidence to support older adults through holistic, person‑centred care—helping improve outcomes for people living with Frailty.

Move with Mary is a series of five exercise videos. They cater for every level of capability, ranging from simple seated stretches, to balance, strength and gentle aerobic exercises suited to older people. The videos have been developed In partnership to support the work of the Northern Ireland Frailty Network, Age Friendly Network NI, care homes and Age Sector Networks delivering locally for the benefit of older people.
The Northern Ireland Frailty Network was launched on the 28th March 2019 by Dr Michael McBride, Chief Medical Officer and Robert Ferguson, a Service User and Information Ambassador/ Peer Facilitator for Age NI.
