Frailty Learning Pathway

To help increase awareness and improve understanding of Frailty, we are delighted to share that we now have a Regional Frailty Education Programme. This programme was developed and tested by the NI Frailty Network and Clinical Education Centre in collaboration with representatives from various HSC disciplines from across all Trusts, independent sector, CEC and primary care.

Frailty Learning Pathway offers a structured approach for all roles and experience levels. The programme builds awareness, skills and confidence to support older adults through holistic, person‑centred care—helping improve outcomes for people living with Frailty.

The programme is delivered within three tiers.

Tier 1: Frailty Awareness is universally relevant to service users, family and carers and all health care staff.

Tier 2: Frailty: An Introduction eLearning (CEC). This is for all HSC staff and pre-registered students and covers:

  • Frailty Awareness
  • Falls Universal
  • Fundamentals of Delirium and Frailty
  • Fundamentals of Nutrition and Frailty
  • Continence Awareness and Frailty
  • Frailty Assessment

Tier 3: All professionally registered staff working within HSC who are working clinically with the older adult on a regular basis. This tier includes:

  • Fundamentals of Frailty
  • Fundamentals of Cognition
  • What Matters Most
  • Fundamentals of Polypharmacy
